The Single Assessment Mechanism for Elderly Services (SCNAMES) is the single entry point for applying for any subsidised long-term care service. It covers both residential care and community care application pathways.
The key assessment points include:
- The assessment scope covers activities of daily living, cognition and communication, mood and behaviour, physical functioning, health condition, social support, home environment, and other areas
- The assessment is conducted at home by a recognised assessor, who may be a social worker, nurse, occupational therapist, or physiotherapist
- The interRAI Home Care 9.3 assessment tool is used (updated from v2.0 in July 2021)
The assessment is free of charge, and the result is valid for 12 months. One assessment can cover both the residential care and community care application pathways at the same time.
